Shift description

This is an assignment in Fort Lauderdale, FL requiring 5 shifts per week. Hours will be roughly 8am-5pm, Monday-Friday. One weekend day shift is required per month. This assignment includes one week of orientation / precepting (5 shifts).

Minimum of 1-year full time experience in Care Management as a Social Worker required to apply.

Requirements

  • Master’s Degree in Social Work from an accredited program.
  • BLS Certification.
  • Minimum of 1 year full time experience in Care Management as a Social Worker in an acute care setting within the last year.
  • Proficient in supporting patients' access to post acute care services and appropriate utilization of resources. Collaboration with post-acute care providers to achieve optimal health and ensure patient needs are met in the community.
  • Must show competency with assessment, discharge planning/intervention, implementation, care coordination, monitoring and evaluation of the plan of care.
  • Proficient in managing a caseload of high risk, complex needs and/or catastrophic patient(s).
